Summer Camp Release Form

Caregiver may give consent that their student can sign themselves out and/or leave with another student at the end of camp day.

This release form is only for students ages 12 to 18 and MUST be filled out 2 Fridays prior to the first day of camp.

Caregivers will receive a call from The Rose Studios to verify the form.
